Please call us and arrange a short visit with a staff member to determine that our services are going to meet your needs. Then, be sure to complete the Trial Day Form and bring it to your visit.
Once you decide that you would like to enroll your family member in our program, we will need a completed enrollment packet along with a Medical Assessment signed by the individual’s primary care physician in order to begin providing services. In addition, our Nurse will spend some time visiting with you and your loved one in order to learn more about his or her interests and health related needs.
